• Name (including middle initial, suffixes)
• Current mailing address (if PO Box is used, the physical street address is also required)
  • Documentation showing current address, if the address on the ID is invalid.
• Date of Birth
• Social Security #/Copy of Social Security Card
• Phone Number
• Valid ID with correct mailing address displayed
